Submission Requirements:
The completed Proposal, Original & Seven (7) copies, including, W9,
Affirmative Action Compliance Notice, Affirmative Action
Questionnaire or Evidence, Equal Employment Opportunity Language,
Americans with Disabilities Act of 1990 Language, Non-Collusion
Affidavit, Owner’s Disclosure Statement, New Jersey Business
Registration Certificate, Certificate of Employee Information
Report, and Chapter 271 Political Contribution Disclosure Form,
must be received by the Business Office no later than Friday,
February 10, 2012, and be addressed to:
